E-DRUG: Coverage on Antiretroviral therapy

Dear E-druggers,
I would be very grateful to get clarification on the following:
1) Coverage on antiretroviral therapy.
Here my interest is to know at what point coverage is calculated. Is it at the
time of initiating patients on therapy (coverage at uptake) or is it calculated
based on active patients (irrespective of how many where initiated on therapy).
What is the role of the SPECTRUM model in the measurement of coverage of
patients on ART? What considerations should be made when one compares coverage
between health facilities.
2- The terms duration and retention on therapy are often used in relation to
patients on antiretroviral therapy. Which of them is the better terminology to
use, why and when should it be used?. What would duration on therapy and
retention on therapy measure assuming that they are used differently?
