E-DRUG: Information-sharing and handover teaching resources?

Dear E-druggers
Can anyone point me to resources that would help with building a culture
of information sharing within an essential drugs program or any health
program? Or for building and implementing strategies for handover?
In some cases there seems to be insufficient appreciation of the
importance of sharing information and talking to each other within
programs. In addtion, it seems quite common that people filling
significant places in the system can be moved away, or might leave for
study or for any other reason and the new person has had no preparation
to fill the posion. Planning for a seamless handover would be ideal.
I would be grateful for any pointers.
