E-DRUG: Annual additions to health budget for new technologies

Dear colleagues,
There have been many publications on the additional funding required to add new health
technologies to national formularies.
Countries such as the UK (Wanless report of 2001) mentioned an annual addition of
approximately 2% for new technologies (drugs, devices, procedures etc.).
This has sometimes been referred to as a "technology index".
Is anyone familiar with actual health policy decisions stating that an annual amount or percentage of health expenditure (about 2%) should be added each year to cover the
cost of new and improved medical technologies?
Is there any relevant literature that you would recommend on this subject?
Any appropriate links, publications and referents on this subject would be appreciated.
