There are no templates as such but there are references which can guide e.g the book " Managing Drug Supply" by MSH, the chapter on quantification of
medicine requirements.
Also you need to collect morbidity data and treatment guidelines used for managing malaria in your area.

"Management of Drugs at Health Centre Level" (WHO/AFR/EDP/04.3) useful.
It explains the different methods for estimating drug needs and demonstrates them with worked examples.
