This is a question with no firm answer, because

(a) the dose depends on the indication, age, body weight,
renal function etc. etc.
Moreover,

(b) especially in the case of antibiotics it is obvious
that DDD is NOT a clinical measurement unit. This has been
made perfectly clear by the WHO centre in Oslo and repeated
many times.

You should use literature data instead. Best wishes from an
old and experienced clinician.
