E-drug: HRT - prescribing guidelines

I am currently investigating the prescribing of Hormone
Replacement Therapy (HRT) in menopausal women for my
MPharm and am therefore interested in sourcing any published
therapeutic guidelines for prescribing of HRT in the management of
menopause.
I have access to the following guidelines:
Consensus report by Lauritzen et al, 1996;
The South African standard treatment guidelines and essential
drugs list for adults (National Department of Health, 1998);
American Association of Clinical Endocrinologists (AACE) Medical
Guidelines for Clinical Practice for Management of
Menopause (Cobin et al., 1999);
AACE guidelines for the prevention and treatment of
postmenopausal osteoporosis (Hodgson et al., 1996);
The Canadian consensus conference on menopause and
osteoporosis (Rowe et al., 1998) and;
The National Health and Medical Research Council of Australia's
booklet for health professionals (1999).
I would appreciate it if anyone could inform me about any
guidelines which have been published elsewhere.
