[As mentioned below updating the EML is crucial. Murtada Sesay (12 in this thread) noted that the list should reflect the up-to-date therapeutic options. I suggest you look at the Fiji EML - updated recently and reflects the recommendations in the National Treatment Guidelines. There is a column indicating the relevant guideline for each medicine. Medicines are on the EML because they are indicated in the treatment guidelines - not just included because someone wants them.

These electronic days it should be easier to keep EML updated to reflect new guidelines. Between actual reprints, electronic updates can be circulated. BS Moderator]

Great discussion on essential medicines. The concept of essential medicines is vital for universal access while the need to regularly update is also important in order to meet the highly dynamic medication needs of the population.

However, if I may share the Kenyan experience, where implementation has been not so good.
In Kenya the situation is such that few of the medicines in the list are actually available, consistently. In essence this has created a "super essential medicines list" and therefore further limiting access. This may be due to limited pharmaceutical financing but beats the purpose of the list.

Another challenge is the updating of the list which takes long and participation by health professionals and the public is minimal. The new devolved system of governance has created another challenge in terms of needs and compliance.

Therefore the access to the medicines in the list is still a major challenge for Kenya, and I dare say,many low-middle income countries. It is important to have this at the back of our minds even as we explore newer definitions of the concept. Thanks.
