E-DRUG: SP(sulfadoxine pyrimethamine)

Dear E-druggers,
In literature on malaria treatment we notice that SP (sulfadoxine
pyrimethamine) or the combination with Artesunate is reserved for treatment
of children and pregnant women. In particular it is mentioned for the
intermittent treatment.
However, the resistance to SP is quite high in some countries and therefore
the results one can expect are rather limited.
1.What is the rational behind this thinking.
2.Who developed this suggestion or reservation of SP or other sulphonamides
for this indication.
3. In an era of broad use of Artesunate based combination therapies
(available in all malaria endemic countries) what is the rational of
reserving this therapy for children and pregnant women.
4. Can we do better with straight ACT's??
