E-drug: UN session tackling African pandemic (cont'd)

In the article sent by Leela McCullough on 23/9 it is said that "...with
the exception of Botswana, less than 1% of HIV-infected pregnant
women in heavily infected countries receive treatment to prevent
transmission of the virus to their children, though the drug is free..."

I really don't know enough to understand this absurdity. Is it because
of procurement problems, maybe especially in rural areas? But in
places where a laboratory rather than clinical diagnosis is made aren't
the drugs available together with testing facilities? Sorry about my
ignorance but can anyone explain this to me?
