I haven't got a quick answer to Jane's second question but perhaps the
reason why a publicly-funded and provided health service is inflexible,
inefficient and not user friendly is that the more successful it is
(efficient, friendly and flexible), the more people want to use it and
so the service gets swamped and staff are overloaded and they become
less efficient, less flexible and much less friendly. A successful pri-
vate service has much more scope to take on extra resources as required
and can more easily maintain a balance between efficiency, flexibility
and user-friendliness. Effectiveness is another matter altogether.
