This announcement has come in very handy. I have a patient who is
scheduled for open Heart surgery for valve replacement in a weeks time.
Not in failure. She is 16 years known but stable sickler. I hope to get
the haematocrit above 35-36 before surgery with reduction of Hb SS to
well below 30%. I unfortunately don't have lab means for quantification
amounts of Hb. The regimen we have been using is as follows:

- day 1: transfuse 250mls packed cells under Lasix; bleed out 500mls
  whole blood 3 hours post transfusion; transfuse 250mls packed cells.
- day 2: transfuse 500mls packed cells under Lasix; bleed out 500mls
  whole blood and transfuse 250mls packed cells.

We do haemogram pre and post preparation and will top up more packed
cells if the expected haematocrit is not attained. The usual theatre /
patient environmental temperature control and use of 100% oxygen pre
and post extubation will, of course, be undertaken.
