[e-drug] ADRs and cox 2 inhibitors

E-drug: ADRs and cox 2 inhibitors
---------------------------------------------
August 24,2000

May I know if any e-druggers have any information about the incidence
of bleeding from the cox2 inhibitors like vioxx? I am asking this
because of a recent incident that happened in our hospital involving
a 15 yr old male patient who underwent an
emergency exploratory lap for abdominal pain which was
diagnosed as acute appendicitis. Postop, he was given vioxx for pain
control. He developed rashes thereafter but was sent home still on
vioxx as home med. He was brought back to the emergency room the next
day where he was managed as a case of anaphylaxis. He eventually
expired after rescuscitative measures
failed. Autopsy report showed pulmonary hemorrhage.

The ADR subcommittee met to discuss the case but representative of
the allergy section thinks it was a case of Henoch Schonlein which
can present as abdominal pain. (Histopath of appendix showed only
congestion). However, with recent reports from Australia about
gastrointestinal bleeding occuring more frequently with the cox 2
inhibitors than expected, I maintained my opinion that we cannot
rule out ADR to vioxx in this instance.

I would appreciate any additional or related
information from the group.
