A reply to Prof. Urquhart's comment on compliance with ARVs:
It seems there is a misunderstanding. Poor compliance with ARV leads
of course to the rapid emergence of resistant strains. What we
wanted to say is that starting ART should be an informed decision by
the patient: the patient should be motivated and think that he/she can
comply with the treatment. This does not eliminate but maybe reduces
problems with compliance later on. This is different from TB treatment
where patients even have been imprisoned to ensure compliance (like
in New York state)!
