E-drug: Hepatitis-B vaccination coverage

Dear E-druggers,
We are preparing for la revue Prescrire, an article on programmes of
vaccination against hepatitis B in the world.
Would you be able to help us with the remaining gaps in our file :
- Among low endemicity countries, we have not found official documents on
the vaccination programmes against hepatitis B in Switzerland, in Belgium,
in Portugal (what about endemicity there), and in the Netherlands. Should
you have access to such documents, it would be nice if you could help us by
sending copies of the relevant pages or give information on how to get
them. We need most recent recommandations for at-risk groups (with
definitions), non-at-risk infants, children or adolescents.
- High endemicity countries. WHO claims that 80 countries have included
vaccination against hepatitis B as routine for children (GPV information
4/12/98). Therefore a good number haven't yet. Which of the high-endemicity
countries have not yet routine vaccination implemented ?
