E-drug: Historical data on antibiotic prescribing

A colleague is doing a project that entails describing the history of the
prescribing of antibiotics internationally, particulary in Africa. Does
anyone have data that might address any part of the following:
* Production levels of antibiotics (volumes and/or dollar value) for the
period 1950 - 2000. (Global, regional or by country)
* Data on the different types of antibiotics (and volumes of production,
sales or use of these) again for the whole world , region or by country.
* Data on the levels of sales, manufacture or use of antibiotics in Africa.
