E-drug: Public enquiries into withdrawals of and safety problems with drugs ?

I would be very grateful to e-drug readers for a quick response
to this question: Is the USA the only country in the world where
public enquiries have been held when serious drug safety problems
have emerged, e.g. leading to withdrawal of products from the
market? I am aware of none in the UK or elsewhere, but I'd
very much like to check.
If you know any such enquiries in any country other than the USA,
I would be very grateful if you could let me have a citation/reference -
or an indication of which drug(s) were involved and when. If you
