E-drug: Recipe per patient

"We are planning to implement a "recipe per patient" (R/P) form in Health
Ministry's primary care settings in Nicaragua. We are convinced that this form
is more useful than the present form because it contains information on all the
drugs prescribed to a patient, and it takes less time for prescribers to fill
it.
At present physicians are using a recipe per drug (R/D), what means wasting
time and paper. And R/D forms make it difficult to carry out drug
utilization studies.
Administrative personnel do not agree with R/P forms because they
increase difficulties in the accounting process.
My questions are:
1. In your country, what kind of form is being used in primary health care: R/P
or R/D?
2. If R/P forms are used, how do you control drug stocks in pharmacies?
