E-DRUG: Request for Model Phamaco-Vigilance System Forms

We would very much appreciate receiving soft copies of any forms used to
solicit and monitor information on adverse drug events, including quality
problems, reactions, inter-actions, apparent resistance, etc. We are
interested in both generic forms and forms specific to drugs/therapies. If
there are any instructions for the forms and/or explanations of how the
information system to track adverse drug events is organized, we would also
greatly appreciate receiving those.
