E-DRUG: Setting up Hospital/ Clinic Pharmacies in Resource-Poor Settings
-----------------------------------------------------------------------------------
Hello,
I would like to ask everyone for a good reference or guideline for setting
up a pharmacy in a hospital or clinic. I was asked to help with planning
the set-up before construction starts.
We will be building in a rural part of Sierra Leone where electricity and
running water are scarce.
Thanks in advance.

[This site leads to the latest edition of the well known 'MSH Yellow Book' and it is worth having the complete book. However all the separate detailed chapters and sections in the book can be downloaded separately. BS]

[The first publication mentioned by Philippa is great and as she says - out of print. Many good old WHO Publications have been scanned and are available but I can't find this one either (from AHRTAG). If someone has a hard copy and has the patience to scan it as a pdf it would be wonderful to share.
The second one (MDS 97) is widely available but details for the new edition were mentioned yesterday in message (2) of this thread. BS)
Dear Suzanna,
It is sad that many useful and practical publications for rural clinics and
hospitals have gone out of print. I have not seen a request such as yours
for many years.
There are two publications on my shelves that you would find helpful, if
you can find them:
1. 'How to manage a health centre store: building, layout, equipment and
managing supplies', published by the Appropriate Health Resources and
Technologies Action Group (AHRTAG) in 1994. Kathy Attawell was the editor.
(You could try Googling her). This guide is very practical, and does not
assume knowledge or resources beyond the basics.
2. 'Managing Drug Supply', published by Kumarian Press for Management
Sciences for Health, in Collaboration with WHO. My copy is old - 1997, but
it there have probably been up-dates. This guide is for more complex
systems, but is very useful. Look at MSH's website.
I hope others will suggest resources for you.
Best wishes,
Philippa Saunders.

Dear Suzanna,
I am not sure if you would yourself be visiting Sierra Leone in the near future. Although I cannot immediately cite a reference, I would invite you to contact me ( kindiatown@hotmail.com), as I am now relocated home in Sierra Leone, and can share past local experience on this subject with you. I have also recently been involved in looking closely at the supply chain of a tertiary health facility in a small Island country.
If you have more specific questions, this would be helpful.
Best of luck in your work.

Dear Suzanna,
Philippa Saunders has mentioned two important publications for your project.
The latest edition of 'Managing Drug Supply' you should get through WHO or
MSH. I happen to have a hard copy of 'How to manage a health centre store:
Building, Layout, Equipment and Managing supplies' in my office. We will try
to scan it for you during the next days and you should have before the
weekend.

Dear Suzanna,
From your post, I understand that you want to create a medication storage facility and a dispensing facility and both do not necessarily have the same design although the concepts are the same.
There was also the 10 session Commonwealth Pharmaceutical Association CPA long-distance training books on drug supply management that are very basic and useful. I have hard copies in storage somewhere, but I guess the one already mentioned will suffice.
I also recommend the 3rd Edition of Managing drug supply that has been highlighted. It is a big publication but you can download and print by chapter as explained in earlier messages.
In addition to all that has been shared, I found this paper on creating safe and more efficient pharmacies.
The local regulatory authority in the country should have its own requirements, so, do give them a visit.
I was looking through FIP and did not find any specific guidance on how to design and build a pharmacy.

Dear Suzanna,
Seems like you have a lot of great material from the others already. You
may also want to consider the Partners in Health 'Program Management
Guide', it notably includes chapters on building site infrastructure and
managing a procurement system which both consider pharmacy issues: http://www.pih.org/library/pih-program-management-guide/
