TERMS OF REFERENCE
UNICEF Supply Division welcomes eligible institutions to submit an
expression of interest to undertake field testings of the Mother-Baby
Pack, an innovative simple, user-friendly and affordable packaged
mechanism to deliver prophylactic antiretroviral (ARV) regimens for the
prevention of mother-to-child transmission (PMTCT) of HIV.
1. Background and Project Summary
In 2008, 45% of pregnant women living with HIV in low-and middle-income
countries received antiretroviral drugs to prevent HIV transmission to
their infants, including antiretroviral therapy for their own health

quite low globally and national programmes are facing various challenges

attributable to weak health systems, frequent stockouts of medicines,
social and cultural obstacles, and many PMTCT candidates are missed or do
not get the full complement of medicines they need.
UNICEF in collaboration with WHO and global partners supporting PMTCT and
paediatric HIV care, support and treatment is developing an innovative
simple, user-friendly and affordable package mechanism to deliver PMTCT
prophylactic medications, the Mother-Baby Pack (MBP). The MBP contains
medicines for prophylaxis, ARVs and Cotrimoxazole, for the mother and for
the infant in line with WHO PMTCT Guidelines (2006) and new
recommendations from November 2009.
2. Purpose of MBP
The MBP is an innovative product aimed to facilitate the scale-up and
increased uptake of more efficacious ARV prophylactic regimens for PMTCT
(combination of Zidovudine and Single-dose Nevirapine) and Cotrimoxazole
prophylaxis in resource-limited settings in line with 2006 WHO Treatment
guidelines and new WHO recommendations from November 2009. The MBP also
intends to help streamline the procurement and supply management of PMTCT
prophylactic medicines.
The initiative takes into consideration limited capacities of health care
systems (institutional and human), operational management including
tracking and follow-up mechanisms for a continuum of care, as well as the
health seeking behaviours of clients.
3. Project Requirements
The full pack is intended to be distributed within ANC clinics to
HIV-positive pregnant woman, ideally at the first ANC visit from week 14
of pregnancy or at the earliest opportunity thereafter. Moreover, some
pregnant women may come later (up to 40 weeks of pregnancy). The MBP will
be also distributed through maternity/delivery facilities to target women
presenting for the first time to the health facility at labour and also
for those seen during pregnancy who received a MBP but will not bring
their pack at the time of delivery.
The MBP is designed to be used both within and outside the clinical
setting. It is intended to be user-friendly, manageable, and
water-resistant. It includes the principal PMTCT commodities in their
basic unit packaging to allow for flexibility of dispensing and
administration to patient.
The objective of the proposed assignment for which UNICEF invites EOIs is
to facilitate the field testing of the Mother-Baby Pack inclusive of its
contents and packaging in the context of PMTCT Programmes, and to provide
performance (effectiveness) and management (efficiency) feedback to the
Mother Baby Pack Technical Working Group with the intention to contribute
to strengthening the effectiveness of the pack itself and its potential
impact on scale-up and increased uptake of more efficacious ARV
prophylactic regimens for PMTCT.
Under the overall guidance of UNICEF and as informed by the Mother Baby
Pack Technical Working Group, the selected institution is expected to work
together with the stakeholders via the UNICEF Project Manager in
field-testing the prototype.
The field testing of the MBP will be conducted in selected African
countries. These field trials are important as it will assess the
introduction of the full MBP (inclusive of its contents) and will measure
its impact on PMTCT uptake. During this process, issues related to stigma,
adherence, supply chain and programmatic issues will be taken into
consideration.
Following the Expression of Interest, UNICEF expects to issue Requests for
Proposals (RFP) in the first quarter of 2010 to all qualified entities who
had submitted EOIs and to other interested institutions.

This EOI does not constitute a solicitation. We do not require bids or
proposals at this stage; we merely seek your expression of interest in
participating in the tender.
A response to this Request for Expression of Interest does not
automatically ensure that you will be selected to participate in the
tender.
UNICEF reserves the right to change or cancel the requirement at any time
during the EOI and/or solicitation process. UNICEF also reserves the right
to require compliance with additional conditions as and when issuing the
final tender document.
