Further to my recent e-mail concerning the project for a Global Di-
rectory of Health Information Centres in developing countries and the
possible funding for upgrading them, we have prepared a version of
the questionnaire which is suitable for completion by e-mail or
printed out, for those whose connection is poor or non-existent. If
you are receiving this by e-mail please fill it in here and e-mail it
back to me directly:
