My suggestion would be to amongst others include a prominent section
on pitfalls of the process, including failure to ensure common
ownership of the documentation, failure of streamlining and
coordinating medicines supply, and most important failure to ensure
proper and reliable mechanisms of obtaining en analysis feedback on
the implementation, misuse, drug availability,
Downreferral of patients and assuring coordinated available
medication to downreferred patient is important.
