E-drug: Improving medicines management. What works best? (cont'd)

Dear e-druggers,
I do agree with Atieno, with regards to Pharmacist on "bedside" of
the patients. This is clinical pharmacy issue and I agree that not
all Pharmacist do experience such unless he/she is a Hospital
pharmacist. Yet, not all hospital require clinical expertise of the
pharmacist and they end up dispensing.
I experienced working in Hospital where the Pharmacist prepare TPN,
IV admixture and monitoring drug administration and physicians
medication orders through the patients profile. For this, knowledge
acquired have not been unused and wasted, specially on
particularities of medicines like stability and expiry of the
medicine upon dilution and frequency of the medication. Pharmacist
have a lot of stock knowledge that needs to be unveiled specially
with patients pharmaceutical care. They also have a lot of knowledge
specially on patient counselling regarding medicine compliance and
best reference for the prescriber who are in doubt, since they also
have deep knowledge in monitoring about different medicines.
Pharmacist serve as control on rightness of prescribed medicines to
the patients.
