Dr Srinavas raises critical issues that suggest he is "on the ground"!
The most exciting part of his contribution for me, however, is rather
squeezed at the very bottom, where he cautiously reminds us that his
comments are made in a personal capacity! I do not know what the
hierarchical structure in his country's Ministry of Health is, but I
assume he is a very senior civil servant. The more such good
examples we have of bureaucrats, so to say, openly challenging and
discussing the numerous obstacles we have to improving access to
essential medicines, the better it will be. Most importantly, the more
such rare breed actually move and are able to change the situation
on the ground, the better still. Unfortunately, and that is why I think Dr
Srinavas' reminder is valid, in countries where access to social
services, including access to basic medicines is least, there is often
much more to it than meets the eye! When will there be 'optimal'
access to basic medicines for all, everywhere?
