E-DRUG: Patients Medication History software

Dear colleagues,
I'm looking for a FREE software tool/database to improve pharmaceutical
care for patients in first-line/second-line hospital settings in
several hospitals in African countries.
First objective is to implement this for the follow-up of patients
under treatment of ARVs, but the broader objective is to use this
database for all patients medication.
I'm very well aware that there are commercial software tools for this
purpose, but all are by far too expensive. Another thing is that those
tools are so sophisticated that software maintanace is needed, which is
not available in the countries where i want to implement this system.
Some of the specification that the database should have:
a) Patients table (incl. adress, age, etc.)
b) Drug table (drug, drug-strength, formulation, etc.)
c) Drug-Interaction table (what drugs have interactions with what other drugs)
d) Drug Contra-indication table (in what conditions should a particular
drug NOT be prescribed)
e) A medication-history overview (prescriptions by patient by date)
f) A built in check for under- and overdosage.
I'm pretty sure such free database-tools already are in use in other
develloping countries, but I just don't know where to find them.
The ideal system should run with Windows/DOS, and should have the specs
mentioned above. But I will be very happy with all
ideas/suggestions/directions and experiences that you might have with
other systems.
It would be absolutelly no problem if you have a tool that runs in
Excel or ACCESS. Stand-alone or via inter/intranet, ALL is wellcome!
I'm hoping for a lot of response, since I feel that the use of these
systems will be the right future for pharmaceutical care to improve
patients-care, medication effectiviness, and to avoid patients risks.
