RFI: Assessment of condom use in the region

Dear Colleagues,
As part of the Ohangwena Poverty Reduction Program (jointly supported
by the UN system in Namibia and the Ohangwena Regional Council), we are
planning to do a relatively small baseline assessment of condom use in
the region. We would like to take advantage of the survey (sample size
probably around 1000, probably ages 15-25 and possibly another older
group) to gather some other information and are in the process of de-
signing a questionnaire. In particular, we would like to include some
questions about the impact of HIV/AIDS on their daily lives (what it
means to be sick for short/long periods of time, how it affects family
organisation, how support structures evolve).
Does anyone have ideas about previously used questionnaires that might
be useful? In particular, I remember a small survey conducted by an NGO
(YRG Care in India) that was very insightful. They interviewed about
100 people living with AIDS and asked very simple yet revealing ques-
tions about the impact of illness on daily life and productivity at the
household level. Of course, now I can't find it when I need it. Does it
ring any bells for anyone? Or something similar done in the region?
Short, simple and concise is what we need!
